Advanced Therapy & Rehab (AT&R) was created by Boulevard Healthcare to provide in-house rehabilitation services within their four skilled nursing facilities located in Illinois and Ohio.  Our large, dedicated rehab facilities contain state of the art Nautilus® equipment specifically designed for the geriatric population.  Treating a high percentage of sub acute patients, including an advanced wound care program, post acute transitional cardiac unit, Alzheimer/Dementia and respite care, our healthcare professionals provide individualized treatments to maximize outcomes and provide compassionate, quality care.
