Our user asked: "I have been disgnosed with a SLAP tear and Biceps Tendonosis in my dominant (right) shoulder. I have excellent ROM and good strength, but pain that flares up when my shoulder is used at different angles. My Ortho surgeon, naturally, wants to cut my shoulder open and fix everything surgically. I cannot personally or financially afford to (essentially) handicap myself for 3-6 months, as I have a physically demanding job and active family life. Trying to avoid massive surgery at ALL costs - that being said, my questions are as follows: (a) what PT exrecises would benefit me at this time? (b) have you heard of Prolotherapy/PRP for shoulder injuries, and if so, are you aware of cases where it benefits people in my situation? Thanks!"

Ask a PT Response: "Labral tears can prove quite difficult to rehab as the function of the labrum is to make the socket (glenoid fossa) deeper so the head of the humerus will have more contact area. Therefore, when the ball (head of the humerus) moves in the shoulder socket (glenoid fossa) when performing activities with the shoulder above shoulder height the head of the humerus may catch where the tear is at the labrum resulting in pain. Rotator cuff strengthening exercises along with GH and ST stabilization exercises are often recommended as well. Yes, prolotherapy may be able to assist with the tendinosis but will not help with the labral tear: http://www.cyberpt.com/COWalrodprolotherapy.asp."
