I'm so sorry to hear that you've been dealing with back pain for so long. It can be difficult to find relief. Have you seen a chiropractor specialist about your problem? I would recommend getting an x-ray of your back, which might identify any potential problems.

Have you tried some PT again? Manual therapy including muscle energy technique, joint mobilizations, traction, STM/MFR, dry needling. Core strengthening & postural training. Maybe cortisone shot and then PT and see how things go? Unfortunately surgery is not a guarantee for alleviation of pain especially since they can not pin point where your issue is.

Looking for some suggestions on path to correct numerous issues... I'm 38 years old. I've been dealing with low back issues since 1999. Over 15 years of PT with some good results, but over last 3 years some newer symptoms with SI joint. Last few months nerve issue is bad down left leg, but pain doesn't get any lower than knee. L1 has old fracture, 10-20% loss of height and slight herniation/bulge. L4-5, S1 herniation. Stenosis in lumbar, minor arthritis as well. Left SI joint has "locked up" a few times. Bad inflammation on left side.
Prednisone only helps small amount. Takes the edge off. Muscle Relaxers have helped for short periods of "flare ups". Getting 3 opinions from different doctors. One went right to cortisone injections. He didn't offer really any guidance other than "Get the shot and see what happens." The talk of surgery will come after the cortisone shot. Chiropractor wants to put me on a 3 day a week program. Meeting with another doctor is scheduled. Any guidance appreciated!
