Our user asked: "I had a cortisone shot last Wednesday after an MRI identified a slap tear. My PT has told me that I can not have surgery until I can free my shoulder up. I had 6 weeks of therapy prior to the MRI & Surgeon visit. Since the shot I have had severe cramping, PT told me this was not cramping. I have repeatedly reinjured my shoulder as I am being told to use it for everyday chores & so forth. I have had no improvement with 6 weeks of PT & a shot. I was sent back to therapy after the shot & still do not have the range of motion I am told I need before I have surgery. Am I on the right path or should I consult for a second opnion?"

Ask a PT Response: "6 weeks of PT and a cortisone injection should have allowed for you to achieve some gains. Have you developed adhesive capsulitis (frozen shoulder) as well? Depending on the size and location of your SLAP tear, this could inhibit your progress as well. I am assuming your surgeon would like you to achieve a certain amount of ROM before the operation. If you have a frozen shoulder, it may be more difficult to obtain the range. Often times the surgeon will go ahead and scope the shoulder and break up the scar tissue or perform a manipulation under anesthesia to assist with helping the patient achieve more ROM when performing the SLAP. If you feel like you need a second opinion, it may not be a bad idea."
