Our user asked: "A month ago, my sister suffered temporary neurological impairment due to a virus that has caused swelling near the base of her cerebellum. She is 24 weeks pregnant, but the baby appears to be fine. After a round of strong steroids and an anti-viral, she has regained movement in her face, but still has extreme fatigue and still cannot walk by herself. My question is this - what can we be encouraging her to do to help in her recovery? Her husband is completely working from home so he can be there 24/7. The rest of us are taking a week at a time and basically living with them. The only time she does any moving around is when her husband walks her from the bed to the couch or to the bathroom. Does she need to be doing more? Is it time to hire a physical therapist to work with her? Thanks so much for any input!"

Ask a PT Response: "I am sorry to hear about your sister. However, I am gald to hear that her baby appears to be fine and that she is beginning to recover from her condition. With conditions that involve neurological impairment, one needs to be careful not to push an individual too hard or it could set him or her back. Exercise should be performed to tolerance and gradually increased over time. Working with a home physical therapist might be extremely beneficial for her at this time especially since she is pregnant and her body will continue to change. The therapist can help develop a plan of care which will assist her in regaining her strength, endurance, balance and function. Meanwhile, I'd recommend your family encourage her to perform a little more activity everyday to her tolerance. For example, maybe walking with assistance around the house for a little while and then increasing the time slightly everyday (not just to get up to go to the bathroom or couch). Also performing lower level exercises when sitting (the PT can help develop this program) can be beneficial as well. I hope this was helpful and please feel free to contact me should you have further questions."
