Our user asked: "Am currently treating a patient with d i s h, the patient spent most of the last 2 years immobile at home but now is trying to follow an exercise program. Has pre-disposing factors [diabetes, over weight, smoker] which are slowing things slightly. His previous physio manipulated his spine and he wants me to do the same, is this recommeded? Thanks"
Ask a PT Response: "Diffuse idiopathic skeletal hyperostosis is a unique form of arthritis and I would be concerned with manipulating your patient's spine given the nature of this condition. If you posess the training to perform manipulations, I would definitely obtain medical imaging of your patient's spine such as an x-ray to determine the extent of the arthritis and what joints are affected in order to give you a better idea if it would be safe to manipulate or not. Maybe you could contact his pevious physio to determine if a true manipulation was performed and to obtain any additional information as well."
