I'm working with a new "back surgeon" who doesn't have a guideline/protocol for his lumbar fusions. I work home health and am trying to develop our own protocol specifically from post op discharge to 8 weeks.  Obviously this group of patients may be able to go to OP when released to drive, many can't.  This MD also refuses to allow lumbar stab of any kind until after 3 months (at which time he d/c's the brace and then starts aggressive lumb. stab.) Any advice is appreciated...most info. is vague.

I would work on: Log Rolling, Bracing Abdominals with movements, general mobility, bed mobility, tranasfers, gait, safety with mobility around the house.
 
If you cannot do L/S stab and because its a fusion and cannot do ROM, then you are not left with a whole lot. Stick to the basics and get the pt moving safely.
 
You can still develop a protocol for exercises, but just not with this surgeon. Also, ask everyother surgeon if your protocol (once developed) is ok with him/her.
