Initial evaluation frequency and duration

   Our user asked: "When an initial evaluation states the duration - 4 weeks (28 days), does this mean a physical therapy treatment can be rendered up to the 28th day and a re-evaluation to follow? My staff is confused, they seem to think a re-evaluation is done on the 30th day (from the initial date or visit the evaluation was completed) meaning 4 weeks make up the month (30 days)? Also, to avoid confusion can a PT write 30 days for a duration?"

Ask a PT Response: "In our clinic, for patients with Medicare, we perform our evaluation and send out the plan of care ASAP for the physician to sign off on. As a precaution we always ask the MD to sign off on 90 days of coverage for PT serivces in case we need it. A two week note and monthly notes are also sent to the physician as well. The patient must also return to see the MD within 90 days as well in order to satisfy Medicare guidelines. This link should give you some clarfication on Medicare billing guideines:
