Our user asked: "I oversee non medical services for a Passport geriatric client. It has come to my attention that there had been a written PROM plan initiated by PT and home health adies were perfroming the exercises. The plan is now lost, and it appears that PT declins to come to the home for an eval and new plan d/t patients previous lack of progress with conventional therapy. The regs state a nurse can write a plan, but I am not comfortable and don't know much about writing atherapeutic plan r/t number of times/day, number of reps etc. Can you give me any direction?"

Ask a PT Response: "Perhaps you could contact the PT again and see if he or she could fax you the previous exercises which the aides had been performing. They should have a copy of the home exercise program in the patient's medical record. Passive Rom Exercises are fairly easy to perform if instructed properly. Otherwise, perhaps you could obtain the services from another PT to conduct an evaluation and develop a plan of care including a HEP. Without actually seeing the patient and having more background on the patient I am not able to suggest a specific plan of care. Good luck."
